We proudly offer a range of services, including specialized support for individuals on the autism spectrum. Whether you’re seeking help for yourself or a loved one, our team is dedicated to creating a safe, understanding, and affirming space where growth and healing can take root.

We offer the following types of therapy to meet a variety of emotional, behavioral, and developmental needs:

  • Cognitive Behavioral Therapy – Our therapy team offers Cognitive Behavioral Therapy (CBT), which focuses on examining the relationships between a person’s thoughts, feelings, and behaviors. During CBT, a therapist will actively work with a person to uncover unhealthy patterns of thought and how they may be causing self-destructive behaviors and beliefs.
  • Dialectical Behavior Therapy – Dialectical behavior therapy (DBT) was originally developed to treat chronically suicidal individuals with Borderline Personality Disorder (BPD). Over time, DBT has been adapted to treat people with multiple different mental illnesses, but most people who are treated with DBT have BPD as a primary diagnosis.
  • Eye Movement Desensitization and Reprocessing Therapy – Eye movement desensitization and reprocessing therapy (EMDR) is used to treat PTSD. A number of studies have shown it can reduce the emotional distress resulting from traumatic memories.
  • Interpersonal Therapy – Interpersonal therapy focuses on a person’s relationships with others, aiming to enhance their interpersonal skills. In this form of psychotherapy, the therapist helps individuals evaluate their social interactions and identify negative patterns, such as social isolation or aggression, ultimately guiding them in developing strategies for understanding and interacting positively with others.
  • Mentalization-Based Therapy – Mentalization-based therapy (MBT) can bring long-term improvement to people with BPD, according to randomized clinical trials. MBT is a kind of psychotherapy that engages and exercises the important skill called mentalizing.
  • Psychodynamic Therapy – The goal of psychodynamic therapy is to recognize and resolve negative patterns of behavior and feeling that are rooted in past experiences. This type of therapy often employs open-ended questions and free association, allowing individuals to discuss whatever is on their minds. The therapist then works with the person to sift through these thoughts and identify unconscious patterns of negative behavior or feelings, as well as how they have been caused or influenced by past experiences and unresolved emotions. By bringing these associations to the person’s attention, they can learn to overcome the unhelpful behaviors and feelings that they caused.

Our team is also proud to offer group and marriage therapy, creating spaces where connection and communication can thrive. Practitioners

Crystal MontagueDr. Crystal Montague, DNP, PMHNP-BC, FNP-C, is a doctoral-prepared and board-certified Psychiatric Mental Health Nurse Practitioner and Family Nurse Practitioner with more than 23 years of diverse nursing and healthcare leadership experience. As the Founder and Clinical Director of Mindful Innovations, Dr. Montague is deeply committed to advancing equitable access to integrated behavioral and medical healthcare for underserved, diverse, and vulnerable populations experiencing significant health disparities and inequities.

Throughout her distinguished career, Dr. Montague has cultivated extensive clinical expertise across the lifespan in a variety of healthcare settings, including inpatient and outpatient psychiatry, primary care, program administration, skilled nursing, home health, and case management. Her clinical approach is rooted in translating evidence-based psychological and medical practices into compassionate, patient-centered care that promotes emotional wellness, improves quality of life, and reduces barriers to healthcare access.

In addition to her clinical leadership, Dr. Montague serves as a part-time adjunct faculty member at North Carolina Agricultural and Technical State University, where she teaches mental health curricula to undergraduate and graduate nursing students. As both an educator and mentor, she is passionate about fostering positive, intellectually challenging learning environments that encourage critical thinking, professional growth, and clinical excellence. She is also dedicated to mentoring future healthcare professionals and mental health colleagues through community engagement, leadership initiatives, and public speaking opportunities.

Dr. Montague is a proud alumna of Winston-Salem State University and the University of South Alabama. Under her leadership, Mindful Innovations has developed clinical affiliations with 26 university Schools of Nursing nationwide, including High Point University, University of North Carolina at Chapel Hill, Liberty University, Gardner-Webb University, Purdue University, Chamberlain University, and the University of South Alabama. Through these partnerships, Mindful Innovations provides valuable clinical learning opportunities and residency support for nursing students pursuing advanced practice education.

Beyond her professional accomplishments, Dr. Montague enjoys reading, writing educational tools and resources for students, spending quality time with her family, and traveling. Her dedication to service, education, and innovation continues to positively impact the lives of patients, students, and communities throughout North Carolina and beyond.

Angel CoxMindful Innovations, PLLC is proud to welcome Angel Cox to our Psychiatric Clinical Team. Angel Cox is a board-certified Psychiatric Mental Health Nurse Practitioner with over eight years of nursing experience in acute cardiac care and behavioral health settings. Throughout her nursing career, she has provided compassionate, evidence-based care to diverse patient populations across a variety of healthcare environments.

Prior to becoming a PMHNP, Angel worked in acute care settings, including ECU Health and WakeMed, where she gained extensive experience caring for patients with complex medical and psychiatric needs. Her clinical background also includes experience in Level I trauma centers, allowing her to develop a comprehensive understanding of the relationship between physical and mental health.

Angel earned her Bachelor of Science in Nursing from University of North Carolina Wilmington and her Master of Science in Nursing with a concentration in Psychiatric Mental Health Nurse Practitioner from Chamberlain University.

As a PMHNP, Angel provides comprehensive psychiatric evaluations, medication management, and brief psychotherapy for adolescents and adults experiencing a wide range of mental health conditions, including anxiety, depression, ADHD, bipolar disorders, trauma-related disorders, and other psychiatric concerns. She believes in a collaborative, patient-centered approach that empowers individuals to actively participate in their treatment and overall wellness.

Angel is committed to creating a supportive, nonjudgmental environment where patients feel heard, respected, and understood. Her goal is to help individuals develop effective coping strategies, improve emotional well-being, and achieve meaningful, lasting improvements in their quality of life.

Outside of clinical practice, Angel remains dedicated to lifelong learning and staying current with evidence-based psychiatric treatments to provide the highest quality care to her patients.

Jeannette RamosMindful Innovations, PLLC, is proud to welcome Jeannette Ramos-Perez to our Psychiatric Clinical Team. Jeannette is a highly dedicated bilingual healthcare professional with more than 25 years of nursing experience delivering compassionate, patient-centered care to individuals across the lifespan. Fluent in both English and Spanish, she is deeply committed to improving access to quality healthcare and fostering meaningful therapeutic relationships with diverse patient populations.

Jeannette’s extensive clinical background reflects a lifelong passion for service, advocacy, and holistic patient care. Throughout her nursing career, she has demonstrated a strong ability to support patients and families through complex healthcare challenges while maintaining a compassionate and culturally responsive approach. Her professional philosophy centers on treating every patient with dignity, empathy, and respect while utilizing evidence-based practices to promote optimal mental and physical well-being.

She earned her Bachelor of Science in Nursing from Colorado Christian University in 2009 and later obtained her Master of Science in Nursing Education from Wilkes University in 2014. Driven by her passion for advancing behavioral healthcare services, Jeannette recently completed her second master’s degree as a Psychiatric Mental Health Nurse Practitioner through Post University. Her advanced psychiatric training has further strengthened her ability to provide comprehensive mental health assessments, medication management, patient education, and individualized treatment planning for patients experiencing a wide range of psychiatric and emotional health concerns.

In addition to her academic and clinical accomplishments, Jeannette proudly served as a commissioned nurse in the United States Army for 10 years, including one military deployment. Her military service cultivated exceptional leadership skills, resilience, adaptability, and a profound dedication to serving others under challenging and high-pressure environments. These experiences continue to influence her compassionate and disciplined approach to patient care and interdisciplinary collaboration.

Jeannette brings a wealth of experience caring for individuals throughout every stage of life and is passionate about integrating clinical expertise, patient education, and mental health advocacy to support healthier individuals, stronger families, and more resilient communities. Her commitment to culturally competent care and bilingual communication allows her to effectively connect with and support underserved and diverse populations, aligning closely with the mission and values of Mindful Innovations.

Tiana GreenMindful Innovations, PLLC, is proud to welcome Tiana Green, MSW, LCSWA to our Psychiatric Clinical Team. She was born and raised in High Point, North Carolina. Tiana has always had a passion for Social Work and loves being able to help others meet their goals and needs. She has experience working with adults and kids in the mental health field. Tiana attended Winston-Salem State University for her BSW and NC State University for her MSW, and she recently obtained her LCSWA in April of 2024.

Ty-shan PickenpackMindful Innovations, PLLC, is proud to welcome Ty-shan Pickenpack to our Psychiatric Clinical Team. Ty-shan is a Licensed Clinical Social Worker Associate with a bachelor’s and master’s degree in social work, specializing in the treatment of depression, generalized anxiety, and PTSD. As a certified DBT practitioner and trauma specialist, Ty-shan supports clients in building confidence, improving self-esteem, and developing effective coping strategies to manage stress and emotional challenges. Ty-shan works with children, teens, and young adults navigating behavioral concerns, family dynamics, and parentified roles, helping them regain balance and emotional well-being. With nearly two years of clinical experience, Ty-shan is committed to creating a supportive space where clients can explore their experiences, strengthen self-awareness, and foster meaningful personal growth.
Dayo PhoenixMindful Innovations, PLLC, is proud to welcome Dayo Phoenix, LCSW, to our Psychiatric Clinical Team. Dayo is a licensed clinical social worker with extensive experience providing trauma-informed, client-centered psychotherapy across diverse settings. Their work integrates evidence-based practices with a strengths-based approach to support individuals navigating anxiety, depression, trauma, and life transitions. Dayo also brings clinical leadership experience, including supervision and program support, with a strong commitment to ethical practice, equity, and high-quality care.

Kia J. HinesMindful Innovations, PLLC, is proud to welcome Kia J. Hines to our Psychiatric Clinical Team. She is a seasoned behavioral health and criminal justice professional with over 20 years of experience supporting adults, children, and families impacted by mental health and substance use disorders. She brings extensive expertise in recovery-oriented systems of care, mental health and recovery courts, intensive case management, diversion programming, and interdisciplinary collaboration. Throughout her career, Kia has worked closely with judicial partners, attorneys, treatment providers, supervision agencies, and community-based organizations to promote accountability, equitable access to care, and sustainable long-term recovery outcomes. Her professional practice emphasizes coordination across complex systems while maintaining a strong focus on individual dignity, strengths, and self-determination. Kia’s work is grounded in client-centered practice, ethical advocacy, and a deep understanding of both behavioral health and justice-involved populations. She is recognized for her ability to navigate complex systems, bridge gaps between courts and treatment, and support individuals and families in achieving stability.

Darline GibsonMindful Innovations, PLLC, is proud to have Darline Gibson serve as our Clinical Manager and Phlebotomist, bringing a strong passion for patient care, leadership, and clinical excellence. With experience in both healthcare management and direct patient services, she is dedicated to ensuring high-quality care and creating a positive experience for both clients and staff.

As Clinical Manager, Darline oversees daily clinical operations, supports staff development, and helps maintain efficient and compassionate healthcare services. In the role of Phlebotomist, Darline is skilled in specimen collection, patient interaction, and maintaining accuracy and professionalism in all aspects of care.

Known for strong communication, attention to detail, and a commitment to teamwork, she strives to make a meaningful impact within the organization and the community served by Mindful Innovations.

Carmen HiattMindful Innovations, PLLC, is proud to have Carmen Hiatt serve as our Executive Director, providing leadership, vision, and strategic direction to support our organization’s mission of delivering compassionate and high-quality services to the community.

Known for strong leadership, dedication, and a passion for helping others, she also works closely with staff, community partners, and families to create meaningful opportunities and improve the lives of those served through Mindful Innovations.
